If you’re caring for an aging loved one or someone with a disability in Arizona, you may have heard about ALTCS—the Arizona Long Term Care System. This Medicaid-funded program provides vital long-term care services to those who qualify, including in-home caregiving, assisted living, and skilled nursing care.

At ABC Care LLC, we often help families navigate this process so they can access care and even get paid to care for their loved ones. Below is a clear guide to understanding ALTCS and how to apply—without the confusion.

What is ALTCS?

ALTCS is Arizona’s Medicaid program for individuals who are elderly, physically disabled, or developmentally disabled and require a nursing facility level of care. It’s administered by the Arizona Health Care Cost Containment System (AHCCCS).

ALTCS covers a wide range of long-term care services, including:

  • In-home caregiving

  • Skilled nursing or assisted living facility care

  • Respite services

  • Medical supplies and case management

  • Personal care assistance (bathing, dressing, mobility)

And more

Who Qualifies for ALTCS?

To qualify for ALTCS, applicants must meet two types of criteria:

Medical Eligibility

You must need a nursing home level of care—but that doesn’t mean you need to live in a facility. Many people receive care at home. ALTCS considers how much help is needed with Activities of Daily Living (ADLs) like bathing, dressing, toileting, eating, and mobility.

A nurse or medical evaluator will perform a Pre-Admission Screening (PAS) to determine your level of need.

Financial Eligibility

ALTCS also has income and asset limits, including:

  • Monthly income must typically be below $2,829 (2024) for an individual

  • Countable assets must generally be below $2,000

  • Exemptions exist for certain assets like a primary home or one vehicle

If income is too high, you may still qualify by setting up a Miller Trust (Income-Only Trust).

How to Apply for ALTCS

To apply, you must first submit a Request for an Application—you cannot start the application directly online.

Here’s a step-by-step guide:

Step 1: Request an Application

  • Download and complete the Request for Application form:
    DE-101 & DE-202 Forms (PDF)

  • Submit your request by mail, fax, or in person to your local ALTCS office.

Alternatively, you can call ALTCS directly at 1-888-621-6880 to start the process.

Note: Avoid using Health-e-Arizona Plus for this process unless specifically instructed. It is primarily used for other programs like KidsCare or traditional Medicaid and may cause confusion.

Step 2: Prepare Your Documentation

ALTCS requires a thorough financial and medical review. Gather:

  • Financial records: Bank statements, retirement accounts, trusts, large withdrawals, property info

  • Medical documentation: Diagnoses, doctor’s notes, medications, care needs

Other items: Marriage certificate, legal documents (like Power of Attorney), vehicle titles, and explanations for financial transactions over the last five years

Step 3: Financial Interview

A caseworker will schedule a financial assessment with you or your legal representative. You’ll need to explain your income, expenses, and assets in detail and submit documentation.

Step 4: Medical Assessment

A medical evaluator will conduct a PAS screening. This determines if you meet the care level required for ALTCS based on your ability to perform ADLs, cognitive status, and medical needs.

This may involve an in-person visit and review of all medical documentation.

Step 5: Eligibility Notification

If you qualify:

  • You’ll receive an official notice of approval

You’ll choose a Managed Care Organization (MCO), such as Mercy Care or UnitedHealthcare, who will manage your care plan and connect you with providers like ABC Care LLC

Important Considerations

  • Approval can take 45 to 90 days from the date of application

  • Backdated coverage may be available once approved

You can apply on behalf of someone else if you are their Power of Attorney or have legal authority
